Social Support Services

BeConnected Social Support Services are funded through the Commonwealth Home Support Program to assist frail older people and their carers to continue to live at home safely and independently.

All referrals for our service must be made through the ‘My Aged Care’ contact centre. Information on how to register with the ‘My Aged Care’ is available on www.myagedcare.gov.au or call 1800 200 422. You can also call our office and we will help you through the process.

For all services there are eligibility requirements and an assessment of your needs will be required. These are done through ‘My Aged Care’ before a referral to our organisation is made. When we receive the referral and you join BeConnected, you can access all services provided by the BeConnected branches.

If you are interested in joining any of our services or activities, we suggest you call our office before contacting ‘My Aged Care’ and discuss your needs with our team. If you like, you can ‘trial’ our service first by coming to activities that are of interest to you. If you would like to become a client, then we can help you to register with ‘My Aged Care’ and complete the referral process.

As a client, you have rights and responsibilities. Your rights include dignified, courteous and non-discriminatory service, the right to choose your service and the right to make a complaint. Your responsibilities include treating our staff, volunteers and clients with dignity and respect, to keep appointments wherever possible and to be responsible for the decisions you make. We provide assistance in many ways. If there is something you need or are interested in doing that is not on our program, please ask us……….if we can’t help, we might know who can.
